Apply for a Reception Place – September 2023

To apply for a place for your child in Reception, September 2023 you will need to complete the following two forms :

  1. Supplementary Information Form (SIF)  click here to download  

          (A paper copy of the form is available from the school office)

      2. Surrey County Council Form can be found on their website here

Supplementary Information Form (SIF)

Once the Supplementary Information Form has been completed, please either email a scanned copy to admissions@cardinalnewmanschool.co.uk, or post a copy to the school for the attention of the Admissions Officer.  You will receive an email receipt from the school, so please ensure you include your email address on the form.

Please note that while completion of the SIF is not mandatory, if a completed SIF is not received, the governors can only consider your application within the last criterion of the school’s Admission Policy (see above)

Baptism Certificates

We will need to see the original of your child's Baptism Certificate, if relevant to your application.  In this case, please post the original to the school Admissions Officer, enclosing a stamped, self-addressed envelope for return and a note of your email address so that we can email you a receipt.
